INCIDENT REPORTING FORM
TASO-SSA Officials that experience incidents at UIL competitions must notify the TASO-SSA Board as soon as possible after conclusion of the match. Use this form to provide the Incident Details to the TASO-SSA Board for review. The board will review your report for items including, but not limited to the following: thoroughness, clarity and the use of proper terminology. After review, a Board Member will contact you with any questions or suggestions of any changes that should be made. Once you have received approval from the Board, you will then submit the official report to UIL via the
